The following forms offer you several options. You can complete the forms online using the tab key to move from field to field plus the space bar or mouse to check boxes. You can then print the form and fax it to the appropriate group. Or, you can download the form and submit the completed form as an e-mail attachment if stated on the form.
All GSA (Civilian) and DoD Federal Agencies in the National Capital Region can become WITS2001 Customers. Assistance is available from the GSA Customer Relations Management Center (CRMC) or Verizon's WITS2001 Customer Service Center (CSC) as noted below.
